Affidavit & Declaration Format
Prepare a self-declaration or affidavit draft with numbered statements and a verification clause.
How to use the Affidavit
- Enter the purpose, such as name change, income or gap year.
- Fill in your name, relation, age and address.
- Write each statement on a new line.
- Add the place and date, download the PDF, and print on stamp paper if an affidavit is required.
Affidavit or self-declaration?
A self-declaration is a signed statement on plain paper. Since the government encouraged self-attestation, many offices, colleges and scholarship schemes accept self-declarations for things like gap years, income, residence or category. An affidavit is a sworn statement, printed on non-judicial stamp paper of the value required in your state and signed before a notary or oath commissioner, making false statements punishable.
Check which one the office needs. Using an affidavit where a self-declaration is enough costs extra money and time.
Common purposes
- Name change, often followed by a newspaper advertisement and a Gazette notification.
- Income declaration for scholarships, fee concessions or EWS applications.
- Gap year for admissions after a break in studies.
- Address proof, date of birth or same person with different names.
- Lost documents for duplicates of certificates or ID cards.
How the format works
The generator creates the standard structure: the deponent’s details (“I, … son/daughter/wife of …, aged … years, resident of …”), numbered statements, and a verification paragraph with place and date. Write each statement clearly and factually, one per line, beginning with “That…”.
Getting it attested
- Print the text on stamp paper of the required value (it varies by state and purpose; e-stamp paper is available in many states).
- Sign in front of a notary or oath commissioner, who stamps and signs it.
- Carry original ID proof; some affidavits need witnesses.
Important
This is a format, not legal advice. Requirements differ by state and purpose, so check with the receiving office or a lawyer. Making a false affidavit is a punishable offence. The document is created in your browser only.
Example statement
“That my name has been wrongly recorded as ‘Rakesh Kumar’ in my Class 10 certificate, and my correct name is ‘Rakesh Kumar Sharma’, and both names refer to one and the same person.”
